Stop payment form – This form should be completed by students if they are wishing to request a stop payment on a refund check due to not receiving it.  The student must wait 14 days after the issuance of the check before a stop payment can be initiated. Parents wishing to issue a stop payment on a parent plus refund check that they did not receive must wait 14 days after the issuance of the check before a stop payment can be initiated. A clear copy of the requestor’s id must be submitted along with the form.

Part time enrollment form- This form should be completed by all part time students to secure enrollment for the semester in which they are registered as part time. Part time students are classified as undergraduate students registered for less than twelve  (12) hours and graduate students registered for less than six (6).
