Matriculation Fee Submission Form
Please submit this form for all credit and debit card matriculation fee payments. This form is for newly admitted students only. Please complete and fax to the Office of Student Accounts at 404.880.6317 or email to email@example.com to be processed.
Stop Payment Form
Students who need to complete a stop payment on a refund check issued should complete the Stop Payment Request Form. It is strongly recommended that students sign up for direct deposit at the time the form is submitted to the Office of Student Accounts. Stop payment should not be completed unless 14 business days have elapsed from the mail date of the check. Once the stop payment request form has been completed, the reissued check will be processed in the following week’s normal refund process.
The Stop Payment Request Form may be completed and submitted to the Office of Student Accounts in three ways:
- In person, at 204 Haven Warren Hall
- By fax , at (404) 880.6317
- By e-mail, firstname.lastname@example.org
- (For fax or e-mail submissions, hand-written signature and a copy of photo identification is required in addition to the completed form.)
