Proposal Routing Form
- form for obtaining internal approval for submission of grants and contracts
After obtaining the required information and signatures of the department head and dean, please submit the completed form to the Division of Research and Sponsored Programs (RSP) with your proposal package. The proposal package should contain the following items:
- Proposal routing form completed in its entirety
- Agency coversheet
- Proposal narrative
- Proposal budget
- Agency certification and assurance forms
- Approval of appropriate compliance committee(s), if required
- Letters of University endorsement, if required
Complete proposal packets should be submitted to RSP no less than 5 business days before the proposal deadline. The proposal should be reviewed internally by colleagues or the proposal review committee of the department, school or center prior to submission to RSP. Signatures of the department head or dean indicate that the proposal has been reviewed and approved for submission.If you have any questions concerning proposal approval and submission, please call (404) 880-6990.
